There's a lot of other stuff that will kill you if injected (most of it much faster and more reliably than HIV). Heck, even an injection of air can do the job. A syringe is a deadly weapon and anyone assaulting me will be treated as such.
I wouldn't wait to find out if it was hiv. Any one comming at me with a gun, screw driver, knife or needle is going to be shot. I don't like shooting or harming someone but I can make an exception.
How to know if the threat is real? The threat is real, sealed if the person wielding the needle acts acts in a manner that proves he's using the threat as part of his "power" over the defender. Seems to me he knows more than I do, thus it should absolutely be taken for what he seems to want me to think: that it's a deadly threat. Entirely likely it's got something in there that could kill me.
I've had a couple HIV/AIDS positive patients threaten to "expose" me or others to their bodily fluids if what they asked wasn't done. Even had to go to court for a fellow nurse who got spit right in the eye by someone who claimed they were positive and the patient laughed about it. Turns out she wasn't, but the scumbag got 6 months in jail for it, if she was actually positive she would have gotten years in prison for her actions. I would rather be attacked by a knife anyday over a needle/syringe. There are plenty bad things a syringe can carry besides bodily fluids that can harm/kill you. Shoot first, ask questions later.
I'm no doctor so I don't know wether it can survive outside the body but I will make it point to ask the EMS when they come to the scene to treat a GSW.
Former EMS guy here, and HIV can live outside of the body, but not for very long if exposed to air (minutes, generally only slightly longer than it takes for the blood to dry). If drawn from a vein into a syringe, much, much longer.
That said, Hepatitis will live for hours (reports up to 48, depending on conditions), is more easily transmitted via needleprick(without actual injection), and will kill you just as dead.
There are also MANY OTHER NASTIES that can mess you up big. Protect yourself if faced with this threat, and possibly even sooner that if he had a knife. An attacker with a knife may be shot and have the strength to stumble to you, but not to attack well. A nut with a needle only need to break your skin ANYWHERE to kill you, even your hand. Like a kamikaze attack, the only defense is to make sure he cannot even reach you while mortally wounded.
A long time ago when HIV and AIDS were fairly new to the lime light and they were starting to learn about it and how it was transmitted and such, there was a man who was deliberately seducing women and sleeping with them just to transmit the virus. Once the woman was found to be positive he would dump her and move onto the next.
It was the first time HIV/AIDS was documented as being deliberately used as a means to intentionally harm and eventually kill.
When the man was caught he was charged with seven counts of attempted murder for knowingly transmitting the HIV virus.
It set a new precedent for needle wielding wackos and sex fiends in that deliberately passing on a deadly virus either through sex or contaminated materials was seen as attempted murder.
If someone is running after you with a tainted needle (or even threatening that it's tainted), by legal terms they are attempting murder and you have every single God-given right to blast the crap out of them. Even if the needle proved to be clean it would be the equivalent of a man pulling out a gun and telling you that it's loaded and that he's going to kill you and you shoot in self-defense and it's later discovered the gun was empty. You had no way of knowing but you felt your life was in danger and you protected yourself. HIV/AIDS is a deadly virus and if used as a weapon the intention is to kill.
It's a Deadly Threat that kills...just a little slower than a fast one.
