I would think some belt maker out there would have addressed this issue. I do know that a company has made fast draw rigs specially fitted for ladies that compete in cowboy shooting. This says that they do know about the issue. Maybe is you called a manufacturer and asked they might already have something or could make you one.

I'd +1 something like a Kahr P9: it's relatively small, certainly thin, the grip is a good size for a smaller person's hand and it's light weight won't pull on their belt.
