Glad you asked this question, I need to know the answer on this one too.

Access to the form is great, but does the state send a reminder, or is the permit holder responsible for getting the renewal in on time without a reminder?

Received a renewal form in the mail yesterday from Utah. They include a link where it can apparently be renewed online.

No Utah will be responsible for you and make sure you know when to renew.
