.40's are the #1 culprit followed by the .45 ACP Glocks. The .40's are by far the biggest problem given the cartridge is already a high pressure round. All it takes is a little lead fouling in the throat of a chamber that causes a pressure spike or a little too much powder and boom... the case head lets go and the gun explodes in the shooters hand.
