Mine is always carried decocked (so hammer at half-cock) with the safety on and a round in the chamber.
One of the things I like about the gun is that it's a very natural motion to draw and flick the safety off. I wear an IWB that presses the gun against my side, so to draw, my thumb starts just above the slide, kind of pressed into the nook created between the rear sight and the edge of the slide. From there, as the gun comes out, I just run my thumb down the side of the gun to the grip, and it catches the safety perfectly every time.

That's what I was going to say. Especially in a stressful situation, we can easily apply more pressure than we think - it could, not only decock so we no longer have a SA trigger, but unless we realize it and release the decocker some, the gun won't fire at all.
That just seems like a bad situation; it seems like if you were going to carry C&L, you may want to convert to V9 as mzmtg mentioned. Of course then you have to carry C&L and cannot decock - just like a 1911.
