I currently have a regular MTAC that I wear for appendix carry. The neutral cant MTAC looks like it would be much better for appendix carry than the regular MTAC.
I am looking for feedback from those that have used both versions.
How do you compare the two for comfort when used for appendix carry?

I am confused about what you mean.
there is only one MTAC. the cant on it is adjustable.

I guess that some could not get a cant that tilted enough for them with the movable mounts.
