New to the forum and been lurking too long.
I'll be interested to see some responses on this.
I have the P9 and typically carry a spare mag loose in my left front pocket.
It's not uncommon to end up with several rounds loose in my pocket. The top round is so touchy that you just need to barely bump it to pop it out. I'm saddened to see that you have the same issue even when using a carrier. I'm ready to give up a really terrific gun because of this.

when i carry my CM-9 in a DeSantis Nemesis pocket holster, i carry a spare mag in a Bill's Custom Cases velcro-sealed pouch that i originally bought to hold a knife. obviously access is slower, but i know that all the rounds will still be in place, and the mag is protected from damage from the other things that i carry in that pocket.

Yes, this is a known issue with Kahr mags.
I make molded leather pocket mag holders for them, they are molded to a loaded Kahr 9mm mag so the top round can not move. You draw by catching the hook on the edge of your pocket.
Here is some recent feedback from the guys on the Kahr forum.

I use a Magazine wallet holster from abetterholster for my PM9 and LCP mags. Carries 2 mags, keeps them upright for fast presentation, and no issue with the rounds popping out.

i saw this, and promptly ordered one. my velcro sealed pouch did NOT keep the top round(s) from exiting the mag, although at least all the loose rounds were in the pouch. i expect BeardogV1's pouch to work better.
