For defensive pistol shooting I try to keep my targets at 25' or less. Rifles ... unlimited really. Depends on range limitations.

I usually practice at 20 yards in for handgun. Sometimes, if the range isn't busy, I'll send the target out to 30 yards or so and then bring it back and fire at it while it is advancing towards me.

For pistol somewhere between 0-25 yards, shotgun 0-50 yards, and rifle 0-500 yards. It really depends on what my goals for the range trip that day are.

I warm up pistol at 3-5 yards, practice a lot at 7 and 10. I believe the statistics say that the average SD draw is at 7 yards, shot is at 3. So that is where I like to practice!

Pistol 3 yds to 25 yds,

Rifle out to 100 yds are as far as the range I am on allows

I'm doing this to some degree as well. I practice the same distances that are required for qualifications for a CCW permit. Here in a Vegas we have to fire:

3 yards - 6 rounds right hand/6 rounds left hand
    7 yards - 2 magazines 6 rounds each
    15 yards - 2 magazines 6 rounds each

There is a one minute time limit for each distance.
