    Did some EMS training on this last night. A medical device that is implanted in the patient's abdomen. It keeps the heart pumping until a transplant is placed. A large white wire protrudes from the abdomen and must stay plugged into batteries which are carried in a vest.

    I've had two pts in my district with LVADs.
    Both times training was hosted by the hospital that placed it.
    Do you do "compressions" on a bladder in the bag squeezed by something resembling a nut cracker?
    Our guys wore theirs in fanny packs.

    I think they had something like this on the “Today” show a while back, isn’t it somewhat like the Jarvic 7 from several years ago but much more compact and portable.

    The new Heart Mate 2s don't use the squeeze pump. It's electricity or nothing. No more Heart Mate 1s being used in my area. They are expecting a revolutionary new artificial heart next year. These are either for heart transplant wait listers or those unable to be listed (end of life therapy).

    The local hospital installing these, Sentara Heart Hospital, put in 17 last year and are expecting 60 total this year. Increased use is driven by insurance approval.

    Items of interest:

    No pulse since it uses a rotary pump. Blood pressure readings useless unless with a doppler system.

    Patients frequently get dehydrated and sick because they are used to a fluid restriction/low sodiuum diet secondary to CHF.

    Strokes are not uncommon.

    There have been several deaths because patients forget to switch back over to house power and go to sleep, sleeping through their low battery alarms.

    They will always have an equipment bag with instructions, including a 24/7 number to their cardiac center. In any emergency, call the number for instructions. EMS may have to transport back to the heart center, even if it is a long way away. Local hospitals often can't handle these patients.

    In the event of a device failure, CPR is a 50/50 proposition. Must be done on the left chest. May dislodge the hoses from the heart, which is a death sentence.
