Actually the bullet may have initially come to rest against his skull and then took five years to work it's way to the surface. I had something similar happen to me. At sometime in my life I stepped on a cactus needle that went deep into my foot and the tip broke off. Years later I started feeling pain on the bottom of my foot when I walked and discovered a small lump. I went to my doctor who performed minor surgery to remove the lump. She discovered the tip of the needle cocooned inside a ball of several layers of skin. She explained to me that my body's reaction had been to wrap the needle tip and then move it to the bottom of my foot and that the whole process took several years.

But the scalp is thin, I doubt it would take 5 years for a 22 to surface.
