Could have had a slow primer,it is recommended that on a "dud" you keep the muzzle pointed downrange for 30 seconds,I usually rack the unfired case out on the ground,but I'm not looking down the barrel of a gun I just pulled the trigger on and heard a click instead of bang.
When I see unsafe gun handling at the range,I will point it out to the person what they are doing and why it is unsafe,sometimes you don't get a chance
