I am a wide guy and I carry SOB daily. I can sit in the car comfortably and I can draw from the SOB position (standing).
I'm no more worried about falling on my back than I am getting in a car accident and I still drive. A matter of fact, I have never fallen on my back in my entire life. You are more prone to injure yourself falling down pocket carry because SOB can only possibly harm you if you fall directly on your gun. Pocket carry can injure you in several positions.
I am so wide I really don't have a choice other than not to carry. I think you need to check yourself on how SOB feels, how does it conceal for you, how is your draw and how concerned are you about the chance you may fall directly back on your pistol and having possibly crush or shatter your spine.
For me, SOB conceals so well that I am not about to change.
