Form 1 is for a suppressor that you make yourself. You pay $200 for the tax and whatever it costs you to make it.

Form 4 is for one that you by at a gun shop,made by a commerical manufacturer that is registered to do so.
The rules are the same except that you must engrave name,address and a serial number that you assign to it on the can.
