Yes. Fingerprints, passport photos, front & back copy of your drivers license, application, training certificate, and $65.25.

I am still waiting for mine. I sent in my app on 3/14. My ATM debited on 3/31, but the transaction description mentions 3/27. If that's the case, I am close.

My check was cashed on 4/7 so i am guessing i have another week or two of waiting

Someone mentioned the front and back of the drivers license. I only sent the front as i did not see anything that indicated both, hope that does not delay me
