In and around the house...
Primary gun, knife, OC spray, and cell phone.

Away from the house...add an extra mag and flashlight.

Carry gun (can vary)
Pocket knife (not necessarily tactical - just useful)
Flashlight (cheap LED; useful for old eyes)
Other - cell phone.
- wallet (ID and CHL)
-Keys (a "guy thing," I guess)
A great assortment of other variables, depending on location/destination. (Including spare mags)
I don't subscribe to the seemingly popular theory of "escallating responses." If I can't address a situation without weapons, I won't waste my time and risk myself (and my charges) with OC spray, tactical-bezel light, extension baton, Tazer, knife,.... If I have a confrontation that I feel warrants a reaction, I want it over NOW. I'm pretty patient and laid back, but if I am pushed to respond and have no non-violent options left, it's now the other party's turn to try harder.
Also, I fear that if TSHFT and I did shoot someone, the prosecutor would nail me for not utilizing every available option (my armory of "Less-Than-Lethal" options) since, after all, I take the effort to carry them. I just don't want the clutter - either on my person, or in court.
Just my humble opinion.

I carry my primary, 2 extra mags and at least one knife. I have a Surefire, but I'm not out at night, so I don't carry it.

I'd like to say I'm better about always carrying a spare mag and a light, but it really isn't the case.

I keep my Maxpedition Versipack Jumbo as my "man bag" which is in the car a lot of the time. There's a spare mag and a light and some Fox OC in there.

The G19 is generally all that's on me, however I do carry a mag and a light now and then. I need to get back in that practice.

Wherever it's legal, I carry some or all of the following items that could be used for SD.

Handgun (12+1) - Strong side
1 Spare Magazine (12) - Weak side
Pocket-knife - Strong side
Flashlight - Weak side
All-metal "Drug Pen" - Weak side
HAK (just ordered) - Not sure where to carry it yet, but thinking under the belt, weak side

My load varies, but from time to time there may be a BUG in the items as well, but generally speaking I'm a one gun guy.
Let us not forget diapers, bottle, baby wipes, etc, etc, etc.

If you ever run out of ammunition, I know for a fact a fully loaded diaper will take out 3-4 BGs.
