my holsters all have it , so i can't comment on what is better but , i tried one without and the slide and safety dug (1911). even my holster on order has one . i also wear a t-shirt between me and it ,for the grips mostly , my outer shirts have suffered from the grips though .

In my opinion, they are more of a comfort thing. I can't stand to have any part of the weapon digging into my side. The only way I won't put one on a holster, is if there isn't any portion of the metal protruding from the top of the holster, such as is with my SP101. Or unless of course, the buyer requests to not have one.

I prefer a sweatguard/sideshield. I really like that my VM2 has one with a little bump that helps to prevent unwanted thumb safety disengagement, especially since I have an ambi thumb safety. I really like that the gun is not digging into my side as well. I also use mine as an aid to removing the gun. After grippong the gun I use my thumb to press against the top of the sweatguard/sideshield. It makes for a much smoother draw.
Please try one with it before you buy one without it. If you dont like it, take it to a boot maker or leathercrafter and have it removed.

I Always Order...

The extra piece of leather on my Fist holsters...a body protector...

Some types of holsters have it, and some must be ordered as an extra option...

In Florida, the warm weather dictates the need for the extra leather to keep the gun away from the skin...it helps a lot...

Stay armed...stay safe!

I think that extra leather acts more as a body shield than a sweat guard. Especially if you have some extra girth like I do. For me, it's more of a comfort thing. Others have hit it pretty well. Comfort, Safety position, shoehorning on reholster, etc...

does for me too, can't stand anything digging/rubbing into me. I order all my holsters with one now.
