+1....bone conduction can come from anywhere in the skull not just the mastoid...in fact with the muscle attachment of the SCM on the mastoid, anyplace on the occiput would work just as well...it just happens that the superior part of the mastoid is close to the canal, and easier for patients to differentiate between sides.

The indoor/outdoor plugs from MidWay coupled with muffs work great, IMO. If you take the muffs off temporarily, you can hear well but you are still adequately protected if a shot rings out unexpectedly.
